Glowmeup is committed to ensuring that your privacy is protected. If we ask you for any information by which you can be identified when using this website, then we assure you that it will only be used in accordance with this privacy statement.
- This policy may change and will be accordingly updated from time to time. Please check this page regularly. This policy is effective from 10/08/2019.
- We may ask for your name, address, phone number, and email address.
- We collect this information for Internal record keeping. We may also periodically send promotional emails about new products, special offers or other information which we think you may find interesting. We may sometimes use your information for market research purposes.
- We ensure that your information is secure. We have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online.
- If you have previously agreed to us using your personal information for marketing purposes and you later change your mind then please contact us by email or writing to us.
- We will not sell, distribute or lease your personal information to third parties unless we have your permission or are required by law to do so.
- If you happen to note that any of your information that we have is incorrect or incomplete then please write to us or email us as soon as possible, at the above address. We will promptly correct/complete the information.
This is an informed consent form that has been prepared to help inform you of the potential benefits and risks of semi-permanent makeup (SPMU). It is important that you read this information carefully and discuss fully with your practitioner before proceeding with treatment.
INTRODUCTION: Semi-Permanent Makeup (SPMU) is a field of aesthetic practice that encompasses microblading, micropigmentation and cosmetic tattooing.
Micropigmentation and cosmetic tattooing however require a specialist tattoo machine to apply pigment. These treatments result in permanent cosmetic results and pigments fade less quickly than microblading. Common areas that are treated include:
- Eyebrow tattoo – to enhance or restore colour, fullness, shape and hairstrokes.
- Lip blush – to shade and colour the lips and reduce the need for makeup use.
- Shaded eyeliner – to shade and colour the eyelid margin and reduce the need for makeup use.
TREATMENT: Before your treatment appointment you will receive a consultation. At this appointment, your practitioner will ascertain your goals and desired expectations, help choose a pigment with you that matches your skin tone, they will check you are medically safe to have the treatment and may draw up a mock shape e.g. for eyebrows. This way you can agree on the shape and style before going ahead.
SPMU can be painful, you will have a local anaesthetic cream applied before the procedure which will help minimise any discomfort. The SPMU procedure will take various time periods depending on complexity, body part, treatment aim and individual factors.
A £50 non-refundable booking fee is required on booking, which is deducted from the cost of the treatment.
Client must inform the clinic if they have had Cosmetic tattooing done before. For refreshes of cosmetic tattooing done elsewhere the artist need to see them first and you’ll be advised on an individual basis. Only if they faded to barely anything will carry out a refresh and a full treatment will be provided.
It is also important that you take as much time as you need to consider the treatment carefully, weighing up all your options before reaching an informed decision. It is essential that you are aware of your right to have a second opinion and you are encouraged to ask any questions that come to mind throughout the entirety of the process.
RESULTS: Results vary between clients and results are dependent on many individual factors. I understand and accept that each procedure is a process requiring multiple applications of pigment to achieve desired results and that 100% success cannot be guaranteed during the first procedure. More than one treatment is usually required to achieve or maintain desired results, most people require at least 2 treatments as a minimum.
RISKS AND SIDE EFFECTS: As with any procedure there are potential risks and complications associated. You must be aware of all the following risks before proceeding with SPMU treatment. You must fully discuss any questions with your practitioner.
Common complications and risks include: pain or discomfort either during or after the procedure. It is common for the skin of the eyebrow area to appear red and swollen afterwards, you also may have some minor bleeding, bruising or scabbing, these effects can last up to 14 days but may last longer, it takes up to 6-8 weeks for SPMU tattoos to heal.
Uncommon complications and risks include: Skin infection (cellulitis) which can present as hot, red, shiny skin, there may be pus and you may also feel generally unwell. Infection is uncommon especially if you follow the aftercare advice. In the event of infection, you must see your doctor as soon as possible. For lip blush tattoos these can sometimes trigger reactivation of cold sores (herpes simplex virus infection). Some people may faint or feel faint with needles, if you feel unwell during treatment you must inform your practitioner as soon as possible. Other uncommon risks include poor cosmetic result, lack of result, rapid fading of colour, inconsistent colour and fanning or spreading of pigment. Cosmetic tattoo pigments can often be permanent, despite the term semi-permanent makeup. They are not easily removed and in the event of poor cosmetic result you may require correctional treatment or even tattoo removal such as laser. You must also inform your doctor or radiologist if you require an MRI scan following microblading as problems can occur with tattooed skin during the scan.
Rare complications and risks include: unsightly hypertrophic or keloid scarring, delayed inflammatory nodule (granuloma) formation, skin reaction to local anaesthetic cream and allergic reaction. Despite a normal patch test, allergic reactions are still possible to the tattoo pigments. Allergy can present as swelling, red rash and sometimes blistering, however rarely anaphylactic reactions can occur. In the event of an anaphylactic reaction, you would need emergency medical attention. In the case of severe allergic or inflammatory reaction permanent scarring may result.
For eyeliner SPMU procedure there is a rare risk of eye injury which may include corneal abrasions or corneal scarring (which may lead to visual impairment). Eye infection or inflammation is also possible. In the event of any eye complications, you must seek urgent medical attention. Eye drops may be used before and after the treatment to help protect your eyes.
Several appointments may be necessary to produce optimal results and will be notified, in advance of each session of treatment, about the location where the next treatment session is going to take place and the identity of who is going to be involved in my care at each stage.
True colour will be seen about 1 month after each procedure, and that the pigment may vary per skin tones, skin type, age and skin condition. Some skin types accept pigment more readily and no guarantee on exact colour can be given. Actual colour of the pigment may be modified slightly, due to the tone and colour of my skin.
Non-toxic pigments are used during the procedure and that the result achieved may fade over a period of 1-3 years. Even once the colour fades, pigment itself may stay in the skin indefinitely. There are no guarantees however on the success or longevity of SPMU treatment.
PUBLICITY MATERIALS: I authorise the taking of clinical photographs and videos. I understand that photographs and video may be taken of me for educational and marketing purposes.
CONSENT: To my knowledge, I do not have any physical, mental or medical impairment or disability that might affect my well-being as a direct or indirect result of my decision to have the procedure done now. I agree to follow all pre-procedure and post-procedure instructions as provided and explained to me by the technician. Failure to do so may jeopardise my chances for a successful procedure. I can confirm that I have received a copy of aftercare details. I have been informed of the nature, risks, and possible complications and consequences of permanent skin pigmentation.
I have been asked what information I want and would need in order to make an informed decision. I have been given the opportunity to discuss my desired outcome fully in order for me to make an informed decision.
I certify that I have read the above consent and that I fully understand it. I have been given ample opportunity for discussion and all my questions have been answered to my satisfaction. No new information has become available that affects my decision to have the treatment or my decision to consent. I hereby consent to this procedure. This constitutes the full disclosure and supersedes any previous verbal or written disclosures.
PATCH TEST
The pigment used for this treatment is of medical grade. It is very unlikely that you will experience an allergic reaction, however it is recommended that the patch test is carried out 24-48 hours before the treatment is carried out.
All clients are offered a patch test. We can carry out a patch test on the skin at least 24-48 hours prior to the procedure, however can be waived. If you do want a patch test please contact Rita so we can arrange a time to call in to the clinic to carry out this out.
Client accept there may be risk of an adverse reaction to this treatment, including the possibility of swelling, irritation and redness. You understand that an adverse reaction can happen at any time and not necessarily after the initial treatment. You understand that if these reactions occur - You should contact therapist immediately. If symptoms persist - You must seek medical attention. You understand that in the case of a reaction - You will not hold therapist responsible in any way.
All deposits and booking fees are non-refundable unless agreed to with the practitioner.
I understand there are risks associated with having this treatment and go ahead at my own risk
I understand as part of the procedure eye irritation, lip swollen, bruises, discomfort and in rare cases infection may occur.
I understand and agree to follow the after care instructions provided by my technician. Failure to follow the after care instructions can cause infection or no result.
I understand that this agreement is binding and that I have read and fully understand all information listed above. I represent that I am over the age of 18 years.
AFTERCARE
SEMI PERMANENT MAKEUP (SPMU) AFTERCARE
The following aftercare advice is essential to help reduce the risk of treatment
complications, to improve your comfort/healing and to help achieve the best possible
results from treatment.
Please follow the aftercare advice for semi-permanent makeup (SPMU) treatment.
- Avoid soaps and cleaners to the treated area for the first 2 weeks, these can
remove pigment. Instead wipe the treated area with a cotton pad and cool boiled
water which is sterile.
- Avoid picking, scratching, scrubbing or peeling the treated area to avoid scarring.
- Avoid sun exposure until the healing process is complete. Wear sun block cream
and for eyeliner SPMU wear sunglasses. Sunbeds should also be avoided.
- Keep the treated area clean and dry.
- Avoid strenuous exercise while the area is healing as excess sweating can
remove pigment.
- Avoid using cosmetic and skincare products to the treated area.
- Avoid makeup to the treated area, makeup can potentially distort the colour and
increase the risk of infection afterwards in the treatment area. For eyeliner eye
shadow makeup can be applied carefully above the implanted line, be careful not
to interfere with the treated area. Mascara can be applied carefully to the eyelash
tips only.
- For eyeliner avoid contact lenses and use glasses instead for 2 weeks.
- Avoid hot showers, baths, saunas and steam rooms as these will cause sweating
which can remove pigment.
- Avoid swimming as chlorinated water can potentially remove pigment.
- Apply the aftercare cream as advised by your practitioner.
- The complete healing process can take 6-8 weeks, however colour remaining
after 2 weeks will be long lasting.
- You must seek medical attention and contact your practitioner if you experience
any signs or symptoms of infection after treatment. Infection can present as hot,
red shiny skin, there may be pus formation and you may have a fever or feel
generally unwell.
- You must seek emergency medical attention if you experience any severe allergy
symptoms after treatment. Symptoms may include rash, facial swelling and
breathing difficulties.
- Contact your practitioner as soon as possible if you notice any other unwanted
side effects.
- Your practitioner will advise you when future treatment appointments and/ or top
up appointments are required. If you are advised to attend a follow up
appointment, please do make every effort to attend them. You should do this even
if you believe that the recovery process is going well and you cannot see that there
are any visible complications.
It is essential that you have read all of the information available. Once you have read and
understood all of the below, please sign the declaration at the bottom of this form. Please
do let your practitioner know if you have any questions or if you do not understand any of
the aftercare instructions provided below.
LATE ARRIVALS
We regret that if you arrive late for your appointment, this may decrease the time of your treatment. If you arrive more than 15 minutes late, we may have to treat your appointment as canceled.
CLINIC ETIQUETTE
We ask that you arrive at the clinic at least 5 minutes early for your appointment so that you are relaxed and ready for your therapist at the appointed time.
MEDICAL DETAILS
On your first visit to Glowmeup, you will be asked to complete a client consultation form. Please remember to keep us informed of any changes with your health or special requirements. Please notify us upon booking if you are on any type of medication.
PERSONAL BELONGINGS
We regret that we are unable to accept any responsibility for the loss of personal belongings in the clinic.
CHILDREN
We as a beauty clinic want to ensure that you and our other clients receive a relaxing treatment. Our therapists are unable to carry out your treatment if they also need to keep an eye on child safety. Younger than 12 years old children are not allowed to the clinic.
PETS
No pets are allowed in the beauty clinic.
BICYCLES
In our clinic, there are not allowed to take a bicycle with you into the clinic. You should leave your bicycle outside the clinic.
